Frequently Asked Questions
Do I need a referral to see a specialist?
No referral needed. You can visit a specialist directly. However, appointment availability depends on the provider's protocol.
What needs pre-authorization?
Pre-approval is required for: all inpatient treatments, MRI / CT / Ultrasound scans, endoscopy, physiotherapy, long-term medication (>30 days), oncology, Vitamin D testing, angiography, and outpatient procedures above RO 50.

Emergencies are exempt — but must be reported to Liva within 48 hours.
How does cashless / direct billing work?
Show your insurance e-card at the hospital reception. The provider will verify your eligibility and bill Liva Insurance directly — you only pay the applicable co-pay. No out-of-pocket payment for the treatment itself.
What if I go to a non-network provider?
Not covered. Treatment outside the Liva / PDO network is not reimbursed. If supported by PDO's medical team, claims may be submitted through the PDO medical e-claim process.
Are pre-existing conditions covered?
Yes — fully covered up to the annual benefit limit. No underwriting requirement or declaration of good health is needed.
Is psychiatric / mental health treatment covered?
Omanis: Outpatient only — max 15 sessions/year.
Expatriates: Outpatient + up to 10 inpatient days/year.
Are sports injuries covered?
Recreational (non-professional) sports are covered:
Omani Employees: Covered; surgeries with 15% co-pay.
Omani Dependents: Not covered.
Expats & Dependents: Covered with 10% co-pay.

High-risk / professional sports are excluded.
What happens in an emergency?
Go to any network hospital immediately. Emergency treatment is 100% covered without pre-authorization. You must notify Liva Insurance within 48 hours of treatment.
